In order to participate in the 2019 NBA Health & Performance Expo, you must complete and upload the following items:

  2. Vendor Finance Form - Premium OR Basic* (below)

  3. One-Page Executive Summary

  4. Sales & Marketing Deck

  5. Vendor Questionnaire

  6. Re-name all files to include your company name before uploading (e.g., CompanyName_Legal)

Standard Vendor ($5,000)

  • Vendor booth

    • 6x6 area with table & two chairs
